Please include these details, as relevant:

  • your name and location
  • date and time of problem (even if approximate)
  • title
  • item barcode
  • patron barcode
  • patron phone number
  • computer name (or a specific location, if name unknown)

Requests for collection reports should be submitted via the Collection Report Request Form.
